This page it for downloading the questionnaire only. It is not an online form. 

Clicking the link below will download the document to your local "downloads" folder. You will need to fetch the document from that location to open in Microsoft Word. You can also upload the document from your local downloads folder to your preferred cloud (e.g. Google Drive, Microsoft OneDrive, Apple iCloud, etc.) from there you can use the cloud document editor to complete the questionnaire.

Kindly print and return this questionnaire to a Consistory member, the box beneath the Communications Center in the parlor, or email to readingtonreformedchurch@gmail.com, by Sunday, October 26th.
